Good operators are not people who make fewer mistakes. They're people who make mistakes differently — smaller, recoverable ones, made faster, with a better system for updating when they're wrong.

The Nine Components, in Pairs
The nine practices aren't a linear checklist — they're organized around the failure modes they prevent. Reading them in pairs shows why:
Information hygiene + Uncertainty calibration. These two work together as the input layer. Most decisions start from incomplete information held with too much confidence. Getting better at both means your decisions start from more honest ground — not certain ground, just honest ground.
Second-order mapping + Loop awareness. These are the forward-view layer. Second-order mapping asks what happens next. Loop awareness asks what's happening now in a way that keeps producing the same outcome. Together they give you a picture of the trajectory, not just the snapshot.
Incentive tracing + Bottleneck identification. These are the structural layer — the places the system creates predictable failure modes that individuals can't fix through better judgment alone. The posts on incentives and bottlenecks (05 and 06) are the most structurally important because they're the ones that survive individual excellence. You can be a great operator inside a badly designed system and still produce bad outcomes.
Reversibility assessment + System boundary-setting. These are the scoping layer. Before you invest analytical energy, these two questions determine how much energy is warranted: how costly is being wrong, and what's actually in scope? Getting these wrong wastes effort on trivia and underanalyses consequential decisions.
Decision review. This closes the loop. Without it, the other eight practices degrade over time — your calibration drifts, your loop awareness atrophies, your second-order maps stop matching reality. The review layer is what keeps the stack from settling.
The Self-Assessment (20 Minutes)
Take 20 minutes. No discussion, no team input. This is your baseline.
Step 1 — Decision audit (6 minutes). List the last five consequential decisions you made. For each: what did you expect to happen? What actually happened? Write one sentence on each. Don't include decisions where the outcome was predetermined or routine — these should be real bets, where the outcome was uncertain when you decided.
After each, note: is the gap from bad luck, bad information, or a flawed model? (Sometimes all three. Name the dominant one.)
Step 2 — Loop check (5 minutes). Name one problem that has shown up more than once in the last year — not exactly the same way, but the same archetype. Write it down. Then write: what's feeding it? What happened right before it happened last time?
If you can't name the loop, the problem is still outside your model. That's data.
Step 3 — Incentive trace (5 minutes). Pick one decision from Step 1 where things went sideways — or one ongoing initiative. Write: who gets rewarded if this goes well? Who pays if it goes badly? Is the person with the most to lose from the downside the same person making the key calls?
If not, that's an accountability gap. It doesn't mean the decision was wrong. It means the incentive structure was set up to obscure the risk.
Step 4 — Reversibility check (4 minutes). Look at your current decision queue — the three things you're actively deciding on right now. For each: if you're wrong, how easily can you undo it? Have you given the irreversible ones the most time and the reversible ones the least?
If the irreversible one is getting the least attention because it feels routine, that's the inversion problem from post 02 — alive in your own queue.
